Important Instruction For
Health Checkup

• Kindly book your appointment with the health check coordinator.
• Please call: 0452 – 2545494 or mail us to [email protected]
• Please ensure that you have been fasting for 12 hours prior to your check up.
• Please report on empty stomach between 7.30am and 8.00am at the preventive health check counter
• If you are already a registered patient of Vadamalayan please remember to bring the registration
  number.
• Please bring your recent medical prescriptions and medical records
• Please share complete and accurate information with your doctor.
• Please take your diabetic and B.P medications as usual.
• Regarding other medication please check with the coordinator through the receptionist.
• Do not take any other liquid apart from water on the day of check up.You will be allowed to have
  food after few important tests are completed.
• Bring your stool sample in a clean container if possible.Preferably arrange to collect the
  containers prior to the check up.
• Kindly inform the coordinator if you are diabetic,pregnant or having a cardiac ailment.
• Please dress up in casual loose fitting clothes.Avoid wearing tight clothes,shoes,socks,etc to
  reduce changing time and inconvenience to others.
• The health checkup examination and routine investigations have certain limitations and may not
  able to detect all latent & asymptomatic diseases.
• Pregnant women or others suspecting pregnancy are advised not to undergo X-Ray test. Please
  avoid any health check during menstrual period.
• Fasting not required for well women and child health check plans.
